Person centred planning / mapping provides a way of helping a person to identify key areas of support and plan how that support can be provided in all aspects of their life.

Person Centred planning enables the person to think about what is important to them in their life. These planning tools are very versatile and can be used either for people you support, personally or when working in a team.

Person centred planning will enhance your existing care skills and adopt new ones in a practical, person-centred way.

Visual tools used include but not exclusive are:

  • PATHS – Can be used to help a person / organisation to look to the future when you feel you are a crossroads or your faced with a complex situation. It enables collaborative partnership working with all stakeholders to reach a desired goal / outcome.
  • MAPS – An alternative planning tool to the PATH, that can create a collective vision of a positive future for the people you support and their families. MAPS identifies the person’s strength, gifts and talents, whilst acknowledging any difficulties that may cause the person to become unhappy.
  • Essential lifestyle planning – Learning how someone wants to live their life, what support do they need, It is combined with other person centred tools to engage with people and work out how to get the life they want.
  • Circles of Support – Who supports me with my needs. Looks at inner and outer circles of support.
  • This is me – On page profiles to help people gain a snap shot of who you are and what is important to you and for you.
